Children bring a lunch from home each day
3s Program – child turns 3 years by 12/31/21 and toilet trained
4s Program – child turns 4 years by 12/31/21
5s Program – child turns 5 years by 12/31/21 OR has not yet entered kindergarten

Registration is not confirmed unless and until application form is received completed in full and accompanied by full payment, and a completed health form is submitted no later than one week prior to child’s first day enrolled in the program.
Payments can be made by credit card either online or over the phone, or by check made payable to
“First Church Preschool”.
Early Bird Rates:
JAN 1st THRU APRIL 9, 2021!
3s Program = $325/wk
4s/5s Preschool Programs = $360/wk
After April 9th:
3s Program = $350/wk
4s/5s Preschool Programs = $385/wk
A medical form is required in order for a child to attend, even if payment has already been received on behalf of that child. The form itself is a standardized, CT State issued Early Childhood Health Assessment Record, which can be accessed online on our website here, or from the preschool office. The form must be completed by a licensed medical practitioner, stating that the child has had an exam 8/7/2020 or later. Further, the child’s immunization records must be included.
For children with serious allergies or medical conditions which require medications, all of the prescribed medication must be supplied in its original packaging and labeled with prescribed instructions. Further, an Emergency Health Care Plan must be prepared by both parents and a licensed medical practitioner, and an Authorization for the Administration of Medication by Child Day Care Personnel form must be completed in full. Please contact us if you have any questions regarding your child’s medications.
WHAT TO BRING:
Bathing suit, towel, water shoes, full change of clothes, and an alternate pair of sturdy shoes for sports activities. Also, sun protection, such as hats, sunglasses, or light jacket.

Sunscreen should be applied at home prior to coming to the program.
Children should bring a lunch from home. All items in the lunch should be free of peanuts and tree nuts, in keeping with our Nut Free School Policy. We provide snacks and water to drink.
